So I have create external PHP page first?

Do you have step by step instruction on how to implement this code use iframe code or links for external page?

The script uses PHP code for front-end integration but WordPress does not allow external php code to be executed so the only way to integrate the script on a WordPress page is:
1. To create a php webpage on your server and to integrate the front-end part on it.
2. Use iframe code pointing to that php page for your WordPress webpage.

We have created simple_review.php page.

Please, check if the code is like the one from your Admin Panel and upload it in your root directory.

After that you can use Iframe code for your wordpress page integration.

PHP Review Script

No, I am sorry, we do not offer translated packages. You need to make the translation yourself. You can use the Import/Export option under Options-->Languages menu in the script administration panel to export the text in a file, translate it and then import back into the script.
